1. Service Agreement

By booking a ride with Explore Medical Transportation, you agree to the terms listed here. We provide non-emergency transportation only. If you are experiencing a medical emergency, please dial 911.

2. Booking & Cancellations

Advanced Booking: We recommend booking 24–48 hours in advance.

Cancellation Policy: Cancellations must be made at least 24 hours before the scheduled pickup. Late cancellations may be subject to a fee.

Wait Times: To remain on schedule for all patients, drivers can wait a maximum of 15 minutes past the scheduled pickup time.

3. Passenger Conduct & Safety

Passengers must wear seatbelts or be properly secured in a wheelchair/stretcher at all times.

We reserve the right to refuse service to any individual who poses a safety risk to the driver or themselves.

Explore Medical Transportation is not responsible for personal items left in the vehicle.

4. Insurance & Billing

Passengers are responsible for ensuring their Medicaid or insurance coverage is active.

If a claim is denied by insurance, the passenger or signing facility may be held responsible for the balance.

5. Limitation of Liability

While we strive for 100% punctuality, Explore Medical Transportation is not liable for delays caused by traffic, weather, or unforeseen mechanical issues. Our total liability is limited to the cost of the transportation provided.

6. Governing Law

These terms are governed by the laws of the State of Ohio.
